On April 13, 2016, a Britten-Norman Islander #P2-SBC crashed short of the runway while landing at Kiunga, Papua New Guinea. All 12 people on board including three children, were killed. The pilot was Australian. The PNG Accident Investigation Commission is investigating.
On March 19 2016-FlyDubai Flight 191, a Boeing 737-800 crashed on a flyby after a failed landing at Rostov-onDon, Russia, in bad weather conditions. All 62 people on board are killed. Flight 981 aborted the first landing attempt. Pilots held a holding pattern for almost two hours before aborting a second landing attempt. The plane climbed to 4,000 feet then descended rapidly and crashed near runway 2 at 47°15′30″N 039°49′6″E
On February 24, Tara Air Flight 193, a Viking Air-built DHC-6 Twin Otter flew into a storm and crashee into a mountain side at Dana, Nepal killing 23 on board. Eighteen passengers were from Nepal. One was from Hong Kong, and one was from Kuwait. The wreckage was found afire after impacting the mountainside.
On October 31, Metrojet Flight 9268 #EI-ETJ, an Airbus A321, exploded in mid-air over the Sinai Peninsula 23 minutes after takeoff from Sharm-El-Sheikh, killing all 224 passengers and crew on board. A terrorist bomb secreted aboard was deemed responsible. Kogalymavia was branded Metrojet. 219 Russians, four Ukrainians, and one Belarusian were aboard. The Russian Federal Security Service announced that it was a terrorist attack caused by a bomb containing the equivalent of up to 2.2 lbs of TNT that detonated during the flight. At the time of the crash, it was owned by AerCap and leased to Kolovia. The plane had flown 21,000 flights.
On February 4,TransAsia Airways Flight 235 #B-22816. an ATR-72, stalled on takeoff and crashed into the Keelung River in Taiwan after hitting the Huandong Viaduct. It also hit a taxi on the road. 43 of 58 passengers and crew were killed. Data revealed one engine had in fact been in idle mode. When the right engine failed to produce enough thrust for its rotating propeller, it transitioned into auto-feathering. An unnamed source was reported the pilot had complained of "engine abnormalities" and requested an urgent inspection of the aircraft shortly before its final take-off but there is no proof.
On April 14, Asiana Airlines Flight 162, an Airbus A320 #HL7762 en route from Seoul, crashed short of the runway. The plane hit a localizer on landing at Hiroshima Airport. All 82 passengers and crew on board survived, but 27 were injured. The transport ministry’s Osaka Regional Civil Aviation Bureau said the crew attempted to land the aircraft in darkness and inclement weather without access to an instrument landing system.
On March 24, Germanwings Flight 9525 an Airbus A320 #D-AIPX, crashed in southern France en route from Barcelona, Spain to Düsseldorf, Germany. The crash was a deliberate act of suicide by first officer Andreas Lubitz. All 144 passengers and 6 crew on board the aircraft died in the crash. The accident inspired new regulations by several countries requiring the presence of two authorized personnel in the cockpit at all times. The crash site is within the Massif des Trois-Évêchés. Those aboard were mostly from Germany and Spain, and some had multiple citizenship.
On July 17, Malaysia Airlines Flight 17, a Boeing 777 en route from Amsterdam to Kuala Lumpur was shot down over eastern Ukraine, killing 283 passengers and 15 crew. Pro-Russian insurgents using a Buk surface-to-air missile (SA-11) fired from territory which they controlled. The Russian government blamed the Ukrainian government. The Ukrainian government stated that the missile was launched by "Russian professionals and coordinated from Russia." The crew were Malaysian. Sixty-eight percent of the passengers were Dutch. The flight had two captains, Wan Amran Wan Hussin from Kuala Kangsar and Eugene Choo Jin Leong Zhū Rénlóng from Seremban, and two copilots, Ahmad Hakimi Hanapi and Muhd Firdaus Abdul Rahim.
On March 8, Malaysia Airlines Flight 370, a Boeing 777 en route from Kuala Lumpur to Beijing with 227 passengers and 12 crew, disappeared from radar over the Gulf of Thailand. The Malaysian Prime Minister announced that the airliner's last position before it disappeared was in the southern Indian Ocean. As there were no places there where it could have landed, the aircraft must therefore have crashed into the sea. The search is ongoing.
November 29 – Lam Mozambique Airlines Flight 470, an Embraer 190#C9-EMC, en route from Maputo International Airport, Mozambique, to Quatro de Fevereiro Airport, Angola crashed into Bwabwata National Park in northern Namibia, killing all 33 people. The preliminary investigation report, according to which Captain Herminio dos Santos Fernandes had a "clear intention" to crash the jet and manually changed its autopilot settings, making this a pilot suicide. On August 15 2010, an Aires Boeing 737-700 en route from Bogota to San Andrés Island Colombia was landing during an electrical storm, and impacted before the runway, breaking into three pieces and also impacting one wing. The wreckage spread across the runway as the plane skidded, losing an engine. A small wing fire ensued but was put out by the airport emergency team. 125 passengers were aboard, and six crew members. One female passenger suffered a heart attack en route to the hospital. 13 injured were airlifted to Bogata, and others were treated on the island.
On July 28, 2010, Airblue flight 202, an Airbus A321 with IAE V2500 engines, registration AP-BCB. On approach to Islamabad in poor weather conditions during monsoon rain and low visibility, radio contact was lost (at 09:45) and the plane impacted the terrain in the Margilla Hills during approach to runway 12 at Benazir Bhutto International Airport (OPRN), Islamabad, Pakistan. All 150 passengers and 7 crewmembers onboard were fatally injured and the airplane was destroyed. The flight was a regularly scheduled passenger flight from Jinnah International Airport, Karachi, Pakistan (OPKC) to OPRN.
On May 22, 2010 at about 6:10 am local time (0040 UTC), Air India Express flight 812 (VT-AXV) from Dubai, United Arab Emirates, a Boeing 737-800 with CFM56 engines, overran the runway into a valley landing at the Mangalore International Airport, India. Of the 166 passengers and crew on board, there were 158 fatalities and 8 survivors. The airplane was consumed by post-crash fire. 2 flight and 4 cabin members commanded by Captain Zlatko Glušica, and first officer H.S. Ahluwalia, Glušica, a British and Serbian national with over 10,000 hours of flying and over 7,500 hours of command experience.
On January 25, 2010, Ethiopian Airlines flight 409, a Boeing 737-800, ET-ANB, crashed into the Mediterranean Sea shortly after takeoff from Beirut-Rafic Hairiri International Airport, Beirut, Lebanon. Night instrument meteorological conditions prevailed at the time of the accident. The 8 crew members and 92 passengers were fatally injured. The airplane was destroyed and there was no fire. The airplane was en route to Addis Ababa-Bole International Airport, Addis Ababa, Ethiopia. The investigation is being conducted by the Lebanon Ministry of Public Works and Transport. A U.S. Accredited Representative from the NTSB has been assigned to assist in the investigation.
On May 12, 2010, at 0600 local time, Afriqiyah Airways flight 771, originating in Johannesburg, South Africa--an Airbus A330-200 equipped with General Electric CF6-8E1 engines, tail number 5A-ONG, crashed short of the runway in Tripoli, Libya. Of the 93 passengers and 11 crew on board. The sole survivor was a nine-year-old Dutch boy. The aircraft was delivered on 8 September 2009, eight months old at the time of the incident. The aircraft had logged approximately 1600 hours on 420 flights. The weather at the time of the crash was cloudy, but with good visibility, and no fog or thunderstorms.
On October 26, 2009, at 1934 universal coordinated time (2134 local standard time), a Hawker-Beech BAe-125-800, Russian registration RA-02807, operated by S-Air, was destroyed when it impacted forested terrain during a go-around attempt at Minsk-2 International Airport (UMMS), Minsk, Belarus. Three crew members and two passengers were fatally injured. The airplane was operating on a flight plan between Vnukovo Airport (UUWW), Moscow, Russia, and Minsk, under Belarus flight regulations. The investigation is under the jurisdiction of the Interstate Aviation Committee (MAK)
On June 29, 2009, about 2251 an Airbus A310-300, 7O-ADJ, operated as Yemenia flight 626, collided with the waters of the Indian Ocean during an approach to land at Moroni, Comoros. Of the 153 aboard, one passenger survived. The aircraft was destroyed. Weather was reported as windy. The last departure point was Sana'a.Owned by International Lease Finance Corporation (ILFC) the aircraft first entered service with Air Liberté on 30 May 1990. After leases to successive operators it was leased to Yemenia in September 1999, re-registered 7O-ADJ and remained in service with them until the accident.
At approximately 0220 June 01, 2009, UTC Air France flight 447, an Airbus A330, registration F-GZCP, en route from Rio de Janerio, Brazil to Paris, France lost contact with ATC over the Atlantic Ocean. Automatic maintenance messages were received, and some wreckage has been recovered. Strong convective weather was in the vicinity. The investigation is being led by the French Bureau d’Enquêtes et d’Analyses (BEA) The US NTSB is participating as the State of Manufacture of the General Electric engines.
The Indonesian Air Force Hercules from Jakarta was on approach to Madiun-Iswahyudi Airport when it hit four houses and then skidding into a field, burst into flames on impact. Reports indicated that the airplane carried 14 crew members and 98 passengers. Antara news agency reported that 15 occupants survived the accident. Kompas.com reported a confirmed number of fatalties of 96 but it is unclear if this means additional injured died, or if the number includes several ground fatalities which were also reported.
On May 5, 2009 at about 0900 central daylight time a Cessna 340, Mexican Registration XB-CUC crashed while returning to land after takeoff at General Heriberto Jara Corona International Airport, Veracruz, Veracruz, Mexico. The pilot requested to return to the airport shortly after takeoff and was cleared to land by the tower. The pilot and 2 passengers were fatally injured and 1 passenger sustained serious injuries. The flight had a planned destination of McAllen, Texas.
On April 11, 2009, at 2305 universal coordinated time (1905 local time), a Cessna 210M, Bolivian registration CP-1373, was destroyed when it impacted terrain shortly after taking off from Concepcion Airport (SLCP), Concepcion, Bolivia. The certificated commercial pilot and three passengers, all Bolivian citizens, were fatally injured. Night visual meteorological conditions prevailed. The flight was operating on a visual flight rules flight plan from Concepcion to El Trompillo Airport (SLET), Santa Cruz, Bolivia. The medical evacuation flight was operating under Bolivian flight regulations.
An Arrow Cargo McDonnell Douglas DC-10 freighter en route from Manaus,AM (Brazil) to Bogota (Colombia) with 4 crew, was climbing out of Manaus, when the center engine made a noise described as an explosion. The crew shut the engine down, initially decided to carry on to Bogota but then diverted to Medellin, where the airplane landed safely.Pieces of the plane fell in Manaus damaging a dozen houses, cars, etc. The operator and airplane are US based so NTSB is handling the investigation along with CENIPA (Brazil)
On March 12, 2009,a Sikorsky S-92A helicopter operated by Cougar Helicopters, impacted the North Atlantic 28 miles east of Cape Spear near St. John's, Newfoundland. Two pilots and 16 passengers were aboard but only one passenger survived. The helicopter was en route from St. John's International Airport (CYYT) to an offshore oil platform in the Hibernia oil field. The pilot made a MAYDAY call due to a mechanical difficulty, and was returning to St. John's at the time of the accident. Visual meteorological conditions prevailed at the time of the accident, and the sea state had 3 - 5 meter swells.
On February 12, 2009, a Colgan Air, Bombardier DHC-8-400, N200WQ, operating as Continental Connection flight 3407 on instrument approach to Buffalo-Niagara International, when it crashed into a residence in Clarence Center, NY, 5 nautical miles NE of the airport. 2 pilots, 2 flight attendants, and 45 passengers were killed, one on the ground was killed.The airplane was destroyed by impact and a postcrash fire.
On February 7, 2009, at 1810 universal time coordinates, an Embraer EMB 110P1, Brazilian registry PT-SEA, operated by Manaus Aerotaxi as a commuter flight, experienced an unknown problem after take-off from the Coari, Amazonas Airport (SWKO) in Coari, Brazil, at 1640. At 1722 the pilot informed approach control at Manaus, Amazonas (SBEG), Eduardo Gomes, Brazil, that he was returning to SWKO, but did not state the reason for that. A witness saw the collision of the airplane with the Manacapuru River, in the Amazon Jungle, and observed the airplane sink right after impact.
The BN-2A Trislander was en route from Santiago de los Caballeros-Cibao International Airport , Dominican Republic to Mayaguana (MYG), Bahamas. An hour into the flight the pilot contacted Providenciales ATC (Turks and Caicos Islands) reporting an emergency. It is assumed that the plane crashed into the sea not long after the distress call. The Asociación Nacional de Pilotos announced that the pilot’s license had been suspended in October 2006.
On Nov 4, 2008, at 1846, a Learjet model 45, registration XC-VMC, impacted terrain and buildings on approach to Benito Juárez Mexico City International Airport , Mexico City, Mexico. 9 passengers and crew were killed. The airplane was destroyed. Initial reports indicated that fourteen people on the ground were killed and 40 people on the ground were injured. The airplane was registered to Secretairia du Gobernacion of Mexico, and operated by Centro de Servicios Cooperatives Aeros. The flight originated from the Ponciano Arriaga International airport, San Luis Potosi, Mexico.
On October 21, 2008 a Cessna 310, Mexican #XB-KPF experienced a loss of power on takeoff to Nuevo Laredo. from Pedro J. Mendez Airport, Ciudad Victoria, Tamaulipas, Mexico. The pilot attempted return. The aircraft impacted level desert right of the runway. The pilot and 4 passengers were fatally injured. Advisers from Cessna Aircraft and Teledyne Continental Motors traveled to Ciudad Victoria to assist in the DGAC Investigation. The engines were field inspected. Engines and propellers were shipped to the manufacturers for exam. The fuel boost pumps were shipped to Cessna.
On Sept 19, 2008, Global Exec Aviation Learjet 60 (N999LJ) overran runway 11 departing CAE Columbia, SC for Van Nuys, CA with crew, 2 pop musicians and staff trailing debris on the runway. Takeoff appeared normal, then sparks were observed. The crew attempted to reject takeoff, continued past the runway blast pad,1,000 foot runway safety area, striking airport lighting, navigation facilities, a perimeter fence and concrete marker posts, crossed a roadway and an embankment road. 2 crew and 2 passengers were fatally injured; 2 passengers suffered serious injuries.
On September 14, 2008, at 0510 local time, Aeroflot-Nord flight 821, a Boeing 737-500, Bermuda registered (VP-BKO) and Russian operated, equipped with CFM56 engines, crashed on approach to Perm airport in Perm, Russia. All on board, including 6 crew and 82 passengers, suffered fatal injuries. The flight originated from Moskva- Sheremetyevo Airport (SVO) in Moscow, Russia with an intended destination of Perm Airport (PEE) in Perm, Russia. The flight disappeared from radar as it was executing a missed approach. The investigation cited inadequate training,spatial disorientation and alcohol.
On September 6, 2008, at 2300 universal coordinated time, a Robinson R-44, Russian registration RA-04180, crashed in a wildlife preserve under unknown circumstances. The accident location was near Kuzmino Village, Smolensk Region, Russia. The helicopter was substantially damaged. The pilot and one passenger were killed, and one passenger was seriously injured.
30-year old Boeing 737-200 registered under YV-102T disappeared while en route from Maiquetia to Latacunga. It was being ferried to a new owner after storage at Caracas with 3 crew on board. The aircraft struck Iliniza volcano, elevation 17,000ft. (Latacunga, Ecuador) There is no evidence of poor weather in the area around Latacunga Airport at the time. All 3 occupants were killed. The plane first flew on June 23, 1978 by Frontier Airlines. It later was operated by United Airlines and Atlantic Airlines de Honduras before changing hands to ConViasa.
On August 24, 2008, at 20:40, an Itek Air B737-200, registration EX-009, serial 22088, crashed returning to Manas Airport. After take-off the flight crew reported that the cabin would not pressurize and they decided to return back to the airport. About 10 km from the airport, radar contact with the airplane was lost. The airplane collided with the ground and was destroyed by impact forces and fire. 7 crew members and 83 passengers were aboard; with 65 passenger fatalities reported. Investigated by Kyrzg Republic accident investigation commission and Interstate Aviation Committee, Moscow (MAK).
On August 20, 2008 Spanair flight JK 5022 #EC-HFP McDonnell Douglas MD-82 from Madrid’s Barajas Airport to the Gran Canaria Airport in Gran Canaria, Spain, crashed just after takeoff from 36L Barajas. 154 died. Of the 172 aboard, 162 were passengers. The aircraft rolled right. Unable to maintain adequate airspeed, it crashed near the runway, breaking into two parts engulfed by an explosion. The pilot had previously attempted and aborted a departure due to a sensor reporting excessive air intake temperature. The temperature sensor was de-activated on the ground, delaying departure an hour.
290 km from the Noyabrsk airport, the Mi-8 RA -22599 UT Air helicopter suffered a catastrophic crash as the crew carried out transport- connected flight. A roter malfunctioned on landing, simultaneously with a port engine fire. Witnesses say the helicopter was on fire before the crash 13 passengers were aboard, 9 of whom died. One sustained severe injury.. The helicopter burned. The flight register was completely destroyed by fire. One black box survived and is being decoded, and the radio transmissions of the crew are being studied by the commission of inquiry. Investigation implies that maintenance seems to be a contributory factor.
The Lancair Legacy was heading south to the runway, made a hard right turn and nosed into the ground with one "loud engine rev" before the engine " died out." The propeller was not under power when it impacted terrain. The wings, wreckage, vegetation, and ground did not indicate signs of a fuel spill. It appeared that the airplane had run out of fuel and stalled on approach, causing it to enter a spin prior to impacting terrain in a near vertical attitude. Cited as cause was the pilot's inadequate preflight planning, which resulted in the loss of engine power.
On May 30, 2008 at 1545Z, a Taca Airlines flight No. 390 Airbus A320-233, Irish #EI-TAF, overran runway 02, at Toncontin International Airport (TGU), in Tegucigalpa, Honduras, on landing. The flight from San Salvador, El Salvador to Miami, FL stopped in Tegucigalpa, Honduras, and San Pedro Sula, Honduras, en route. The airplane departed the runway and came to rest on a street. 134 persons were aboard. The captain and two passengers sustained fatal injuries. 2people who were in cars on the street, incurred fatal injuries. The piot landed going the wrong direction, too far down the runway.
On May 24, 2008, at 0920 Pacific daylight time, an Aerospatiale AS-350-D helicopter, N67GE, operated by Island Express Helicopters, Inc., impacted terrain while landing at Two Harbors, Avalon, located on Santa Catalina Island, California, during a 14 Code of Federal Regulations Part 135 on-demand, commercial flight. The flight departed from the Queensway Bay Heliport, Long Beach, California, at 0907. The commercial pilot, a company employee, and one passenger were killed, and three passengers were seriously injured. The helicopter was destroyed by impact damage and fire
On April 3, 2008, Aero JL Cessna 182 registration XB-AIL was en route to Jalapa, Veracruz, Mexico After losing altitude, the Cessna crashed in the Toluca Valley in Xalatlaco, Estado de México, Estado de Mexico
45 year old Ricardo Medina died in the crash; Jose Adrián Hernandez, the pilot survived with injuries. The injured were airlifted to ABC Hospital in Mexico City.
Santa Bárbara Airlines ATR 42-300 Flight 518 departed Mérida (MRD) at 16:59 on a domestic flight to Caracas. Shortly after take-off the airplane flew into the side of a mountain at a height of 4100. The flight recorder indicated that the crew departed Mérida with inoperative navigation equipment and subsequently became disoriented in the mountainous terrain surrounding the airport, crashing into the side of a mountain while trying to determine their location. There were 46 fatalities.
